Welcome, New Patients

Everything you need to get started with care.

Two steps to starting care

There are two steps to starting care: a brief phone call to see if we're a good fit, followed by a full psychiatric evaluation if you decide to move forward.

Introductory Phone Call (15 minutes)

Schedule a complimentary 15-minute phone call with the PMHNP. Choose a time below that works for you.

Watch your email for a confirmation. You'll receive a message confirming your appointment and setting up your client portal account.

This call isn't a diagnostic appointment—it's simply a chance for us to get to know each other, answer your questions, and make sure we're a good fit before moving forward.

What You Should Have on Hand for This Call

  • Insurance plan name
  • For minors: a parent or legal guardian must be present (not required for ages 14 and up)
  • Any questions you'd like to discuss

Initial Psychiatric Evaluation (1 hour)

If you decide to move forward with Shoreline Support & Wellness after your phone call, you'll complete your new patient intake forms through the client portal—this is where medications, psychiatric history, and records will be collected—then schedule your full initial evaluation.

Appointment Expectations

  • In-person visits at our Kailua office are recommended for initial evaluations, though telehealth options are available throughout Hawaii if preferred
  • Treatment plans are developed collaboratively
  • Follow-up frequency is determined together, based on your needs

Prescribing Policies

We are a benzodiazepine-free practice

Shoreline Support & Wellness does not prescribe benzodiazepines (for example, alprazolam/Xanax, lorazepam/Ativan, clonazepam/Klonopin, or diazepam/Valium) for any condition. This applies to new prescriptions as well as prescriptions started by another provider.

There are effective, evidence-based alternatives for anxiety, panic, and sleep, and we will work together to find the approach that fits you best. If you are currently taking a benzodiazepine, please let us know before scheduling so we can discuss whether this practice is the right fit for your care.

In-person visits are required for controlled substances

For any controlled substance we do prescribe, an in-person visit is required before the medication is started, and at least once every twelve months thereafter to continue it.

Visits between those in-person appointments may be completed by telehealth when clinically appropriate. Because these medications require ongoing monitoring, prescriptions cannot be provided without an established treatment relationship and an up-to-date in-person evaluation.
